In this role, you will work closely with patients and their families to provide a holistic approach to mental health, wellness, and social support. Assess the psychosocial status of patients and families to understand their needs and challenges.
Develop and implement comprehensive care plans in collaboration with healthcare teams.
Provide counseling and support to patients and families, helping them navigate healthcare processes and personal challenges.
Act as a liaison between patients, families, and various healthcare departments to ensure coordinated and effective care.
Document patient cases including assessments, treatments, and progress.
Participate in training sessions and seminars to stay updated with the latest practices in social work and healthcare provisions.
Strong interpersonal and communication skills
In-depth knowledge of social theories and practices
Ability to handle complex situations with empathy and professionalism
