Description

Aveanna Healthcare seeks an In Home Healthcare LVN to provide high-acuity nursing care to pediatric and adult patients in their homes. You will deliver skilled assessments, medication and treatment administration, ventilator and tracheostomy care, and close monitoring of complex conditions. The LVN collaborates with RNs, therapists, and families to implement care plans, ensure safety, and educate caregivers. Provide high-acuity skilled nursing care to pediatric and adult patients in their homes
Perform comprehensive assessments and ongoing monitoring of complex conditions
Administer medications, treatments, and therapies per physician orders and care plans
Manage ventilators, tracheostomies, and other advanced clinical equipment as needed
Document care accurately in electronic health records and maintain regulatory compliance
Educate patients and families on disease management, medications, and home care techniques
Collaborate with RNs, therapists, physicians, and caregivers to coordinate care
Follow infection control, safety, and emergency response protocols in the home setting
Participate in ongoing training, skills checks, and competency evaluations
